You would need to obtain three letters of recommendation, from teachers who have taught you between your 8th to 12th grades.   As an undergraduate we recommend that you obtain at least two of your recommendations specifically from an English teacher, and a Mathematics teacher.

To assist your teacher with preparing your recommendation letter, you can provide her a copy of your mark sheets and a list of all your co-curricular achievements and a draft format.
